Transition Learning Collaborative
The Transition Learning Collaborative is a learning approach that brings together partners representing various disciplines, agencies, families, youth and recognized experts in topic areas where they want to make improvement. These team partners learn from each other by targeting those areas that need improvement, regularly sharing their progress through specific projects, and collaborating throughout the process on mutual goals. Team partners provide assistance to one another in terms of challenges, successes, and lessons learned via conference calls, webinars, and face-to face meetings, when possible.
The Transition Learning Collaborative team has split into three work groups. These groups work to ease the transition from youth to young adulthood by focusing on specific areas: Integration of disciplines, Medical Provider Issues, Resource Identification. In addition, working closely with the Pa Youth Leaders, the Transition Learning Collaborative will create materials to encourage Self -Advocacy skills which is intended to help youth seamlessly transition into adulthood by taking charge of their medical treatment and independence.
Once created all materials will be piloted in a minimum of three medical practices to ensure their effectiveness. Welcome to Transition - Draft and Survey
Our Welcome to Transition document has been created by the Integration of Disciplines workgroup. Our hope is that this document would be handed out to a family or young adult by a friend, medical or social service provider as a first step in the planning of transition to adulthood.
